Photo and Recording Release

Kairos Youth Choir needs permission to use your child’s photographs, and/or voice recordings for marketing and publicity purposes. This includes: our website, local newspapers, radio/television stations, promotional brochures, and professional recordings. Please fill out and sign this form as to whether or not you give permission. If you are not comfortable with the use of your child’s individual photo, please know that we may still use group photos with all choir members for social media and our website. If you have any concerns please just send us a quick email.
